Please call the office prior to your appointment if you have questions about your insurance.

If necessary,approval of the Primary Care Provider or Physician should be obtained before an appointment. You may need to obtain approval from your insurance carrier before your appointment.

Please call the office with questions about pre-authorization for care.

Patient Records

Please bring test results or health records to the office at the time of your appointment. Bring a list of medicines used currently. Call the office with any questions about these items. The staff will help you decide what is needed. Your family physician can’t release records without your approval.

Questions About Your Care

The doctor will answer any questions you have about your care at the time of your visit. Family members may be at your appointment to help or assist you and also are encouraged to ask questions.

Patient Records and Correspondence

The patient's physician is provided a report about the appointment if permitted by the patient. A release must be signed by the patient before records are released. Records will be sent by digital technology. Paper based records and charts are not used. We support green office technology. Paper document production requires 2 weeks minimum notice. A maximum allowable charge will be assessed for production.
